Up to 86% of dry eye cases are caused by meibomian gland dysfunction (MGD). Learn what MGD is, how it's diagnosed, and how ProVision Eye Associates treats it with advanced therapies in Blue Bell, PA.
What Is Meibomian Gland Dysfunction?
Meibomian gland dysfunction (MGD) is a chronic, diffuse abnormality of the meibomian glands — the tiny oil-producing glands that line the upper and lower eyelids. These glands produce meibum, an oily substance that forms the outermost layer of the tear film and prevents tears from evaporating too quickly.
When meibomian glands become obstructed or their secretions change in quality, the tear film becomes unstable. Tears evaporate faster than normal, leaving the ocular surface exposed and irritated. This is called evaporative dry eye, and it accounts for approximately 86% of all dry eye disease cases.
Symptoms of Meibomian Gland Dysfunction
MGD symptoms often overlap with other eye conditions, which is why it is frequently misdiagnosed or undertreated:
- •Burning, stinging, or gritty sensation in the eyes
- •Redness of the eyelid margins
- •Crusty or sticky eyelids, especially in the morning
- •Blurry vision that clears with blinking
- •Sensitivity to light
- •Contact lens discomfort
- •Eyes that feel tired or heavy
- •Paradoxical watery eyes (reflex tearing in response to dryness)
How ProVision Diagnoses MGD
At ProVision Eye Associates in Blue Bell, PA, we use a multi-modal diagnostic approach to evaluate meibomian gland health:
Meibography uses infrared imaging to visualize the meibomian glands directly. We can see the size, shape, and dropout (loss) of glands — information that is critical for staging MGD severity and tracking treatment response over time.
LipiView Interferometry measures the lipid layer thickness of the tear film and captures blink dynamics. Incomplete blinking is a major contributor to MGD and is often missed without this technology.
Tear Osmolarity Testing measures the salt concentration of your tears. Elevated osmolarity is a biomarker of tear film instability and correlates with dry eye severity.
Meibum Expression involves gentle pressure on the eyelid margins to assess the quality and quantity of meibomian gland secretions. Healthy meibum is clear and oily; in MGD, it may be thick, toothpaste-like, or absent.
Advanced MGD Treatment at ProVision
ProVision offers the most comprehensive MGD treatment program in Montgomery County:
Optilight IPL (Intense Pulsed Light) by Lumenis is the only FDA-approved IPL device for dry eye disease. IPL uses precisely calibrated pulses of light to reduce inflammation around the eyelid margins, liquefy thickened meibum, and improve gland function. Most patients complete a series of 4 treatments spaced 2–4 weeks apart, followed by maintenance sessions.
Radiofrequency (RF) Therapy applies controlled thermal energy to the eyelid tissue, heating the meibomian glands to liquefy obstructions and stimulate collagen remodeling. RF is often combined with IPL for enhanced results.
Dynamic Muscle Stimulation (DMST) uses microcurrent technology to stimulate the orbicularis oculi muscle, improving the mechanical expression of meibum with each blink. DMST addresses the neuromuscular component of MGD that other treatments cannot reach.
Low-Level Light Therapy (LLLT) / Photobiomodulation uses specific wavelengths of red and near-infrared light to reduce periocular inflammation, improve mitochondrial function in gland cells, and accelerate tissue healing. ProVision uses the Celluma PRO device for LLLT treatments.
Thermal Pulsation and Manual Expression — Following heat application, manual expression of the meibomian glands clears obstructions and restores normal secretion flow.
At-Home Maintenance: We provide personalized at-home protocols including warm compresses, lid hygiene, omega-3 supplementation, and prescription medications when indicated.
Why MGD Treatment Matters
Untreated MGD is progressive. Meibomian glands that are chronically obstructed eventually atrophy — and once gland tissue is lost, it cannot be regenerated. Early diagnosis and treatment preserve gland function and prevent irreversible damage.
If you have been suffering from dry, irritated eyes and artificial tears provide only temporary relief, MGD may be the root cause. Schedule a comprehensive dry eye evaluation at ProVision Eye Associates in Blue Bell, PA today.
