What Is Demodex Blepharitis?
Demodex are tiny mites that naturally live in the hair follicles and oil glands of human skin — including the eyelash follicles. In small numbers, they're harmless. But when they overpopulate, they cause Demodex blepharitis: chronic inflammation of the eyelid margins that can significantly affect eye comfort, vision, and tear film quality.
Demodex blepharitis is far more common than most patients (and many doctors) realize. Studies suggest it affects up to 45% of adults — and the prevalence increases with age.
Signs and Symptoms of Demodex Blepharitis
Classic Signs
- •Cylindrical dandruff (collarettes) at the base of eyelashes — the most specific sign
- •Eyelid redness and swelling along the lash line
- •Itchy, irritated eyelids — especially in the morning
- •A gritty, foreign body sensation in the eyes
- •Crusting around the lashes upon waking
Associated Symptoms
- •Chronic dry eye that doesn't fully respond to artificial tears
- •Blurry vision that fluctuates throughout the day
- •Recurrent styes or chalazia (eyelid cysts)
- •Sensitivity to light
- •Eyelash loss or misdirected lashes
How We Diagnose Demodex at ProVision
Diagnosis is clinical — we examine the eyelid margins and lash bases under magnification using our slit lamp. The presence of cylindrical collarettes (waxy, sleeve-like debris at the base of lashes) is pathognomonic for Demodex infestation.
In some cases, we may epilate a lash and examine it under microscopy to directly visualize the mites.
Treatment Options at ProVision Eye Associates
Xdemvy (Lotilaner Ophthalmic Solution)
The first and only FDA-approved treatment specifically for Demodex blepharitis. Xdemvy is applied twice daily for 6 weeks and works by paralyzing and eliminating Demodex mites. Clinical trials showed significant reduction in collarettes and eyelid erythema.
ZEST (Zocular Eyelid System Treatment)
An in-office treatment using okra-based gel to deep-clean the eyelid margins, physically removing mites, debris, and biofilm. ZEST is comfortable, takes about 15 minutes, and provides immediate improvement in eyelid hygiene.
NuLids Pro
A gentle, motorized eyelid hygiene device used in-office to exfoliate the eyelid margins and reduce the Demodex population. Often combined with ZEST for comprehensive treatment.
Tea Tree Oil-Based Lid Hygiene
At-home maintenance using tea tree oil-based eyelid cleansers (such as Cliradex or Ocusoft Lid Scrub Plus) helps suppress Demodex between professional treatments.
Why Treating Demodex Matters
Untreated Demodex blepharitis doesn't resolve on its own. Over time, it can:
