Demodex mites live on nearly everyone's eyelids — but in some patients they overpopulate and cause chronic irritation, dry eye, and styes. Learn about diagnosis and treatment at ProVision Eye Associates.

What Are Demodex Eyelid Mites?

It may sound alarming, but microscopic mites called Demodex naturally live on human skin — including your eyelids and eyelash follicles. In small numbers, they're harmless. But when they overpopulate, they cause a condition called Demodex blepharitis: chronic eyelid inflammation that is one of the most underdiagnosed causes of dry eye, eye irritation, and recurrent styes.

Studies suggest that Demodex infestation affects 45–65% of adults — and the prevalence increases significantly with age. Despite how common it is, most patients have never been told they have it.

At ProVision Eye Associates in Blue Bell, PA, Dr. Kyle Capel and Dr. Stella Park are experienced in diagnosing and treating Demodex blepharitis using the most advanced, evidence-based treatments available.

Signs and Symptoms of Demodex Blepharitis

Demodex blepharitis can cause a wide range of symptoms, many of which overlap with other eye conditions:

  • Chronic itching of the eyelids and lashes — especially in the morning
  • Cylindrical dandruff (collarettes) at the base of eyelashes — the hallmark sign
  • Red, inflamed eyelid margins
  • Burning, stinging, or foreign body sensation
  • Dry eye symptoms — blurry vision, fluctuating vision, discomfort
  • Recurrent styes or chalazia
  • Difficulty wearing contact lenses
  • Crusty or sticky eyelids upon waking

How Is Demodex Diagnosed?

At ProVision, we examine your eyelid margins under high-magnification slit-lamp biomicroscopy. The presence of cylindrical collarettes — waxy, sleeve-like deposits at the base of the lashes — is pathognomonic for Demodex infestation. We can often visualize the mites themselves in severe cases.

Treatment Options at ProVision Eye Associates

ZEST (Zocular Eyelid System Treatment)

An in-office deep-cleaning procedure using a specialized okra-based gel that physically removes Demodex mites, biofilm, and debris from the eyelid margins. Most patients notice significant relief after a single session.

Xdemvy (Lotilaner Ophthalmic Solution 0.25%)

The first and only FDA-approved prescription eye drop specifically for Demodex blepharitis. Xdemvy kills Demodex mites directly and is used twice daily for 6 weeks. Clinical trials showed significant reduction in collarettes and mite counts.

NuLids Mechanical Lid Hygiene

A gentle, motorized device for at-home eyelid margin cleaning that disrupts the Demodex lifecycle and maintains the results of in-office treatment.

Integrated Dry Eye Treatment

Because Demodex blepharitis frequently co-exists with meibomian gland dysfunction and dry eye disease, we often combine Demodex treatment with IPL OptiLight, Forma-I RF, and Celluma LLLT for comprehensive results.

Can Demodex Come Back?

Yes — Demodex mites are a permanent part of the skin microbiome. The goal of treatment is to reduce the population to a level where they no longer cause symptoms, and to maintain that with ongoing lid hygiene. We'll create a personalized maintenance protocol to keep your eyelids healthy long-term.
